*5. Foreigner Physical Examination Form

The original copy should be kept by the applicant. The form is printed by Chinese quarantine authority), which can be got in the Chinese Embassy in the home country, applicants can also take a medical check-up in line with the items required. The medical examinations must cover all the items listed in the Foreigner Physical Examination Form. Incomplete records or those without the signature of the attending physician, the official stamp of the hospital or a sealed photograph of the applicants are invalid. Please the appropriate time to take medical examination as it is valid for only 6 months.

A recent passport-sized photo of the applicant

*8. Undergraduate school transcript

Documents in languages other than Chinese or English must be attached with notarized translations in Chinese or English.

*9. Bachelor's degree diploma

Graduation certificate in languages other than Chinese or English should be translated into Chinese or English and be certified by notarization.

Two letters of recommendation in Chinese or English from professors or associate professors.

Tips:
For applicants who are in China: If you are an employee, you still need to provide employment separation certificate and your current residence permit. If you are a student, you need to provide transfer certificate, leaving certificate, certificate of attendance rate issued by previous school and your current residence permit.
